Reports overview
Review results recorded when Guides, Forms, and other supported work are completed.
Reports are review records created when supported work is completed, such as a Guide run, Form submission, assignment, schedule, or reminder. A Report keeps the result together with its subject, recorder, source, answers, evidence, and available files so the work can be reviewed later.
Open Reports
Select Reports from the active CareMap workspace. If you can use more than one personal or organization workspace, confirm that the correct workspace is active before you search for a result.
The Reports destination has two views:
- Reports lists individual Guide and Form results you can currently view.
- Aggregates combines visible Reports into private snapshots for trends, summaries, and answer distributions.
Reports are access-scoped. The list, filters, saved views, and detail pages show only information your account is currently allowed to read. Filter options do not expose hidden subjects, Guides, Forms, or folders.
What a Report can show
Open a Report to review its detail. Depending on how the work was completed, the Report can include:
- Overview metadata, including About, Recorded by, relationship, submitted/start/end times, source, status, AI summary controls, export history, delivery history, and evidence drill-down;
- Answers from a Guide run;
- Form submission answers, uploaded files, and score or grading details;
- Events from the Guide lifecycle, navigation, routes, outcomes, and answer activity; and
- JSON and PDF export actions when evidence visibility allows them.
Report detail is for review. It does not let you edit captured answers or rewrite who a completed Report is about.
Understand subject, recorder, and source
Every Report separates:
- About, the person or account the result describes;
- Recorded by, the signed-in user who completed the work; and
- Source, how the work was launched or why it is visible, such as a direct run, assignment, schedule, reminder, Guardian relationship, care team, or organization Staff context.
These labels help you verify that you opened the right record. They do not grant access to unrelated Reports or make the Report shareable by themselves.
Find the right Report
Use the Reports list to sort by submitted date, About, workflow, source, or summary. Use filters for workflow type, subject, folder, Guide, Form, and submitted date range. Saved views let you return to a common filter set.
On smaller screens, Reports appear as cards instead of the desktop table. The same attribution, workflow, source, and submitted-date information is preserved. Use Load more when the list has additional visible results.
If a saved view no longer matches visible data, CareMap shows a notice. Reset filters or choose a broader saved view to confirm whether the Report is still available to you.
Understand access and recovery states
You may see a Report because it is self-recorded, delegated through a Guardian relationship, visible through a care team, or available through an organization workspace. Some evidence can still be hidden by current consent or access rules. When that happens, the Report explains that answers, events, Report slices, observation values, AI summaries, or export actions are unavailable.
If an expected Report is missing:
- Confirm the active workspace.
- Reset saved views, filters, search, and date ranges.
- Check whether the work was actually completed or submitted.
- Review the expected About, Recorded by, and Source details.
- Ask the appropriate person, Guardian, care team, or organization manager to review access if the Report should be visible.
If a completed Report has the wrong subject or recorder, it cannot be edited in place. The recorder can void an active Report when that action is available and then complete the work again with the correct About subject. Voided Reports remain visible as evidence but are excluded from later derived results.
Reports can contain protected care information. Export or share them only through approved workflows and store downloaded copies securely.
