Answer evidence and media
Review Report answers, evidence requirements, and supported media.
A Report preserves the answers and allowed supporting media recorded during a Guide or Form. Report detail is for review; it does not provide controls for editing a captured answer.
Open captured answers
- Open Reports and select the Report you want to review.
- For a Guide Report, select Answers. For a Form Report, select Form submission.
- If a Guide included a Form, use both tabs to review their separate results.
The number beside each tab shows how many answers it contains. The tabs remain usable on compact screens by scrolling the tab row horizontally.
Read a Guide answer
Each answer card shows its Field method, Guide location, capture time, question, and recorded answer. It can also show help text supplied by the Guide author. Choice answers can retain their visible icon or image.
Only answers configured for inclusion in Reports appear. If an expected answer is absent, it may not have been captured or the Guide author may have excluded that Field method from reporting.
Review evidence and media
An answer can show a Required or Optional evidence badge. Its status can be satisfied, missing, pending, failed, or unavailable. Required evidence that is not satisfied should not be treated as complete.
- Photo and whiteboard evidence appears as an image preview when an authorized image is available. Select the preview to open the image in a new tab.
- Signature answers show the saved signature and the signer name when one was recorded.
- Form uploads appear in Form submission as an image or file link. Form sections can also show Not answered, grading, and quiz results.
On Overview, Evidence drill-down groups the Report's answer, outcome, and route records. Answer entries show their evidence requirement and whether projected media is unavailable. Use Answers or Form submission for the clearest item-by-item review.
Understand access limits
You can review only evidence included in a Report you are allowed to open. Consent or access rules can show Evidence unavailable and hide answers, events, Report slices, observation values, AI summaries, and export actions. The notice on the Report explains the applicable restriction; changing tabs or devices does not bypass it.
If evidence is missing or unfinished
- No answers. means a finalized Report contains no reportable Guide answers. No answers yet means the session has not recorded them yet.
- For pending media, keep the Report open briefly or reopen it after processing has had time to finish. CareMap refreshes media status while the Report is open.
- For failed, missing, or unavailable evidence, confirm that you opened the intended Report and read any consent notice. Report detail cannot replace missing evidence; ask the person responsible for the workflow or workspace for the approved next step.
