Answer evidence and media

Report evidence preserves captured answers and supporting files for review. It does not let you edit the recorded response.

Review a Guide result

1. Open the Report and check About and Recorded by.
2. Select Answers for Guide responses. Use Form submission for Form responses and grading.
3. Review Visited field coverage, then each answer's question, response, Guide location, capture time, and any author-supplied help text.
4. When multiple answers are present, use Answer order to choose Capture time or Authored Guide order.
5. Inspect evidence badges and open available media. Use original downloads when you need the saved file.

Answer ordering changes only the view. Authored Guide order uses positions saved during capture, not today's Guide layout. Repeated responses to the same method remain chronological. Answers without saved positions follow known positions in capture-time order; if none have positions, authored ordering is unavailable.

Understand field coverage

Coverage describes reportable fields on visited pages:

• Answered: an accepted response was recorded.
• Explicitly skipped: a Skip response was recorded.
• Answered and skipped: both response types exist for that field.
• Left unanswered: the visited field has no accepted reportable response.

It does not list unvisited pages or prove the entire Guide was completed. If any visited page lacks a captured field list, CareMap says unanswered fields cannot be determined. This is common with older records; do not interpret an unavailable coverage list as zero unanswered questions.

Read the saved response

Text retains captured line breaks. Date/time answers can show the recorded local value, time zone, offset, and UTC instant.

Options retain their saved labels and nested paths, including a partial path if that is what was recorded. Available option images or icons can appear. A Guide's Result Image is playback feedback, not saved answer evidence.

Only reportable answers appear. No answers. on a finalized Report means there are no reportable Guide answers; No answers yet describes an unfinished session without recorded answers.

Interpret evidence and AI labels

Evidence badges identify Required or Optional photo, signature, or drawing evidence. Their displayed states include Provided, Missing, Pending, Failed, and Unavailable. Required evidence that is not Provided needs attention; a badge does not certify the content's accuracy.

An eligible text or transcript response may include an AI-assisted communication interpretation, labeled Non-authoritative, beside Original response. The original remains authoritative. Pending, failed, or skipped interpretation does not remove it. Use Try interpretation again only when offered; follow the displayed retry or credit message. For missing media, wait for processing or reload the Report. For a restriction, follow Report access. Report detail cannot replace lost evidence. Review Events and evidence drill-down for the recorded activity around an answer.
