Void an incorrect Report

Mark a recorded result invalid with a reason, understand retained evidence, and safely create a replacement.

Voiding marks an incorrect Report as invalid while retaining the original record and your reason. Use it when a submitted result has the wrong About subject or recorded answers and your workflow calls for a replacement.

Report detail cannot edit saved answers, About, or Recorded by.

Who can void a Report

The current Void Report action is available to the signed-in account that recorded an active Report. Permission to review a Report as a folder editor, Guardian, care-team member, or organization Staff member does not make this action available.

If you are not the recorder, contact the recorder through your usual process. Do not assume that an administrator or Guardian sees the same action.

Void and replace the result

  1. Open Reports and select the intended record.
  2. Check About, Recorded by, submitted time, and captured answers.
  3. Select Void Report.
  4. Enter a reason explaining why the record is invalid. A nonblank reason is required and is limited to 1,000 characters.
  5. Select Void report and wait for the updated detail.
  6. If appropriate, complete the Guide or Form again with the correct About subject and answers. This creates a separate Report.

You can cancel before submitting the void action. While it is pending, wait for the result rather than closing the dialog.

What changes

The Report remains available to authorized reviewers with a Voided notice, the reason, and the void time. Related observations and encounters are marked voided, and the Report is excluded from new active-Report derived results.

A voided Report cannot generate a new AI summary or use Send to me and delivery retry actions. Available CSV, PDF, and JSON downloads can still export the historical record. Voiding does not retract copies already downloaded or delivered. An existing aggregate can retain the old result until rebuilt; use Check source changes and Refresh to review it.

There is no restore or undo action in Report detail. Do not use voiding merely to hide a result from the inbox; it remains part of the record.

If voiding fails

The dialog keeps the reason and displays the failure. Resolve the connection, sign-in, or access problem before retrying.

If the action may have succeeded but refreshing the detail failed, reopen or refresh the Report and check for Voided before submitting again or creating a replacement. If Void Report is absent, confirm that you are the original recorder and the Report is still active.
