Report subjects and recorded-by details
See who a Report is about and who recorded it.
CareMap keeps two identities on a Report: who the Report is About and who it was Recorded by. These names may be the same, but they describe different parts of the record.
Understand About and Recorded by
- About identifies the subject of the Guide run or Form response. It can be you or an eligible person connected to you.
- Recorded by identifies the signed-in person who completed the Guide or submitted the Form. Choosing someone else under About does not replace the recorder's identity.
For example, if Alex completes a daily check-in about Sam, the Report is About Sam and Recorded by Alex.
Choose who a Report is About
CareMap starts a normal Guide or Form response with Me as the About subject. While completing the work, use the About: name control to choose another available person before you finish or submit.
The list includes only people you are currently authorized to record Reports about. A contact normally needs to allow both View my Reports and Create Reports about me, while effective Guardian authority can also provide the required access. Scheduled work may already identify its About subject.
If scheduled About access is no longer available when the work opens, CareMap may warn you and use your own identity instead. Review the About control before submitting.
Check Report attribution
In Reports, the About column shows the subject name with Recorded by name beneath it. Mobile Report tiles show the same attribution. You can sort the inbox by About when you need to group Reports by subject.
Open a Report and use Overview to review:
- About and Recorded by;
- Relationship, such as Care-linked for a linked Person subject or Self-recorded for a self/account subject; and
- Source, which describes how the work was launched, such as directly, by assignment, schedule, or reminder.
The source is separate from attribution. It does not change who the Report is about or who recorded it. Depending on your access, the Reports inbox can also show why a Report is visible to you, such as Self, a Guardian relationship, a care team, or an organization Staff context.
Historical names and access
Report attribution is saved with the completed record. A historical Report may therefore preserve the name used when it was recorded. If an associated account has been deleted or its original attribution is unavailable, CareMap can show a label such as Deleted CareMap user or Unknown recorder instead of an active profile.
Seeing a name on a Report does not grant access to that person's other Reports or Record. Report visibility still depends on your current self, Guardian, care team, organization, or other authorized access.
If the attribution is wrong or unavailable
- If the intended person is missing from the About picker, confirm the contact is active and has granted the required Report permissions. Ask that person or the appropriate Guardian or workspace manager to review access.
- If a Guide or Form is still in progress, change About before completing or submitting it.
- A completed Report's About and Recorded-by identities cannot be edited. If the attribution is wrong, the recorder can open the active Report, choose Void Report, provide a reason, and complete the work again with the correct About subject. A voided Report remains visible as evidence but is excluded from later derived results.
- If Void Report is not available, contact the recorder or an authorized person who manages access rather than creating a replacement under an uncertain identity.
Treat subject and recorder names as sensitive Report information and share them only through an approved workflow.
