Report subjects and recorded-by details
Check a Report's About subject, recorder, source, relationship labels, and historical attribution.
About identifies the account or Person described by a Report. Recorded by identifies the signed-in account that recorded it. If Alex completes a check-in about Sam, the Report is About Sam and Recorded by Alex. Choosing Sam does not replace Alex's recorder identity.
Choose and verify About
- Start the Guide or Form and inspect About: name.
- Choose the intended available person before finishing or submitting.
- If scheduled work supplies an About subject, check it rather than assuming that it matches the signed-in user.
- After submission, open the Report and verify About and Recorded by.
Normal direct work starts with Me. The picker includes authorized subjects. A contact normally needs to allow both View my Reports and Create Reports about me; effective Guardian authority can also allow recording. Organization membership alone does not make every member an eligible subject.
If scheduled About access is unavailable when the work opens, CareMap can warn you and fall back to your own identity. Check the displayed selection before continuing. See Form filling and Guide playback.
Read the Overview
The inbox shows the About name with Recorded by beneath it. Open the Report to read its Overview introduction. CareMap identifies self-reporting only when the recorder and subject account identities match, not merely their names. It does not infer a role such as therapist or parent.
If identity cannot be resolved, the introduction uses saved labels without claiming self-reporting. Its date uses submitted time when available, then an end time or creation time. An absent AI summary does not hide this introduction.
Expand Session details to inspect:
- Timing: Submitted, Started, and Ended.
- Attribution: Recorded by, About, and Relationship.
- Activity: Source and answer/event counts.
Session details starts collapsed. Expanding it does not generate an AI summary or spend credits. A Care-linked relationship identifies a linked Person. The Self-recorded relationship label can also appear for account subjects; use the actual About and Recorded by identities to determine whether they match.
Distinguish source from access
Source records how the work began, such as directly, through an assignment, or through a schedule. It does not change the subject or recorder. A separate visibility reason can explain why you can review the record, such as Self, Guardian, care-team, or organization access.
Historical attribution can retain saved names. Deleted or unavailable identities can appear as Deleted CareMap user or Unknown recorder. A name on one Report does not grant access to that person's other records.
Correct a mistake
While work is in progress, change About before submitting. A completed Report's identities cannot be edited. The recorder can void an incorrect Report, give a reason, and record a replacement when appropriate.
If the intended subject is missing, review the contact's Report permissions or the applicable Guardian or workspace relationship. For missing historical results, follow Report access.
