Fill and submit a Form
Choose who a Form is About, complete its sections, submit answers, and open the resulting Report.
Complete a Form to record structured answers and create a Report. Work through its sections, check who the response is About, and submit once it is ready.
Complete a response
You need access to an active, ready Form in a workspace that permits new responses.
- In Library, open the Form's actions and select Fill Form. An available schedule or reminder can also launch it.
- Check About: Me or the displayed person's name. Select it to open Who is this report About? and choose the intended subject.
- Read the current section's instructions and answer its questions. Use View Form details in the header to read the full name and description.
- Select Next to continue. Use Back to review earlier sections; the step indicator shows your position.
- Wait for all file uploads, review your answers and About selection, then select Submit Form on the last section.
- Wait for Form submitted. Select View report to inspect the saved result, or Return to Library.
The Report records you as the recorder even when it is About someone else. Automatic grading, when enabled, appears on the success screen; see validation and grading.
Choose the right About person
The picker offers Me and eligible contacts. Recording About another person requires their Care-linked reporting permissions or applicable Guardian authority. Being in the same organization alone does not grant this selection. See contact permissions.
If a scheduled subject is no longer authorized when the Form starts, CareMap can open it About you and display a warning. Review that warning and the header before answering. If you cannot select the intended person, resolve access before submitting.
A submitted Form marks a scheduled occurrence Done only when its saved About subject and launch details match the current date. Submitting About yourself does not complete another person's scheduled work. See schedule completion matching.
Resolve field issues
Required fields and the author's limits apply. An optional field can still contain an invalid partial answer. Read the message beside it, correct the value, or clear an optional answer completely where the control allows it.
Some errors disable Next; others appear when you leave the field or try to continue. CareMap focuses the first invalid field. Final submission also checks earlier sections.
Wait until Upload fields show ready files. Transfers hold navigation and submission. Follow upload answers to replace or remove a selection.
Keep the response open
Unsubmitted answers are held in this open Form; there is no saved-response resume list. Closing, reloading, changing accounts, or losing the page can lose them. The builder's Saved drafts feature does not save responses.
Use Exit Form to leave. After edits, Cancel keeps filling and Discard ends the unfinished response. If discard fails, the response stays open.
For interruptions, follow submission recovery. If the definition changed, use Review updated Form in this response.
Preview uses the field flow but creates no Report or score. Finish Preview exits; chosen files still upload. See Preview limits.
