Form validation and grading
Understand required fields, answer rules, scores, and passing behavior.
Validation keeps incomplete or invalid responses from being submitted. Quiz grading is optional: a Form can collect answers without a score, award points, or report the percentage of automatically graded fields answered correctly.
Resolve Form issues before filling
You need edit access to configure a Form. In the Form builder, open Form issues to see anything that prevents the Form from being filled. Select an issue to return to the affected section or item.
A Form is not ready until it has a name, at least one section, and at least one field or content item. CareMap also checks that:
- every section has a title and every field has a label;
- choice fields have labeled choices, unless the field allows Other;
- image content has an image and alternative text;
- upload fields allow at least one upload type; and
- every automatically graded field has an answer key.
Use Preview after resolving the issues to confirm that the Form works as expected.
Set required fields and answer rules
Turn on Required in a field's settings when the person must answer it. When someone selects Next or Submit Form, CareMap shows the first invalid field and keeps the Form on the section that needs attention.
Required and field-specific rules work together. For example, a required checkbox must be checked, a required date range needs both dates, and a required upload needs a completed file. Text length, number range and step, date or time limits, choice selection limits, and upload count limits are also checked before submission. An Other choice counts only when its text is not blank.
Choose how the Form is graded
- In the Form builder, open Form settings.
- Select Quiz, then choose a Quiz mode:
- Off collects responses without a score.
- Points uses the point value configured for each automatically graded field.
- Percent correct weights every automatically graded field equally.
- Edit each field that should affect the score.
- Under Grading, choose Automatically graded, set the correct answer, and, in Points mode, set a positive point value.
- Leave fields that should not affect the score as Ungraded, then select Done.
Upload fields are always ungraded. An ungraded answer is still saved in the submission and Report; it is only excluded from the score.
Understand answer matching and scores
Automatically graded answers receive either full credit or no credit. CareMap does not award partial credit within a field.
- Short and long text answers ignore capitalization by default. CareMap also trims the answer and treats repeated whitespace as one space. Turn on Case sensitive when capitalization must match.
- Number answers must have the same validated numeric value as the answer key.
- Single and multiple choice answers must contain the configured choices exactly. Multiple-choice order does not matter, but missing choices, extra choices, or an Other response make the answer incorrect.
- Checkbox answers must match Yes or No exactly.
- Date and time answers must match the configured value exactly.
In Points mode, the score is the earned points out of the possible points. In Percent correct mode, it is the number of correct automatically graded fields divided by the total number of automatically graded fields. An unanswered optional field that is automatically graded still counts as incorrect, so use Ungraded for optional questions that should not lower a score.
The score appears after submission and is preserved in the Form Report. Current Form quiz modes do not define a passing threshold or produce a pass/fail result; they report the calculated score only.
Fix common problems
- The Form cannot be filled: Open Form issues and resolve every listed item.
- Submit Form does not continue: Check the highlighted field's Required state, limits, and answer format.
- An answer key issue appears: Add a correct answer or change the field to Ungraded.
- A score is lower than expected: Check the quiz mode, point values, answer keys, and any unanswered optional fields that are automatically graded.
- No score appears: Confirm that Quiz mode is not Off and at least one supported field is set to Automatically graded.
