Form submissions and Reports
Review submitted Form answers in the appropriate Reports view.
A successful Form submission creates an immutable Report. The Report preserves the Form labels, answer choices, order, uploaded files, and grading details that were present when the response was submitted, so later Form edits do not change the completed result.
Open the Report after submitting
- Complete the Form and select Submit Form.
- On the Form submitted screen, review the score when the Form is graded.
- Select View report to open the new Report, or Return to Library to leave it for later.
Previewing a Form does not create a submission or Report. A Form filled directly creates one Form Report. When a Guide is mapped to a Form, completing the Guide creates one combined Guide + Form Report rather than two separate Reports.
Find a Form Report later
- Select Reports in the workspace where the Form was completed, then open the Reports tab.
- Open Report filters and choose the Form by name. You can also narrow the list by subject, folder, or submitted date.
- Select the matching row on desktop or Report tile on mobile.
The Form workflow type finds direct Form Reports. A combined Guide + Form Report is listed as a Guide workflow, but it can still be found with the specific Form filter.
Review submitted answers
A direct Form Report has Overview and Form submission sections. A combined Report also has Answers and Events for the Guide. Open Form submission to review the captured sections, answers, uploaded evidence, and quiz score or grading details when applicable.
Submitted answers cannot be edited. If you recorded the wrong subject or an incorrect response, use Void Report when that action is available, then complete the Form again. Voiding keeps the original Report visible as evidence but removes it from later derived results. Archiving the Form does not remove its existing submissions or Reports.
Access and missing Reports
Form Reports are available to the submitter, Form folder owners and editors, and people who already have access to the selected About subject. View-only folder access does not reveal another person's submissions. Current consent or access rules can also hide evidence and disable export actions.
If an expected Report is missing, confirm that submission reached the Form submitted screen, switch to the correct workspace, reset Report filters and date ranges, and check the selected About subject. If it is still missing, ask the appropriate folder owner, Guardian, care-team member, or organization manager to review your access.
