Import and export Forms

A .form archive transfers a Form definition, including answer keys, cover, display media, and supported settings. It does not contain submissions, completed answers, fill history, or Reports.

Export requires edit access to the source Form. Import requires edit access and authoring capacity in the destination. A package contains the answer key even when that key is not visible to ordinary respondents.

Export a Form

1. Find the active Form in Library.
2. Open its actions menu and select Download to open Export Form.
3. Wait for Succeeded, then review the package size and asset count.
4. Select Download .form and complete the device's download or save flow.

Package history shows recent jobs. Refresh checks the current export and history without starting another export. If Automatic updates paused appears, use Refresh; the notice does not mean that the export was canceled.

If a failed export still has attempts available, Retry retries that job. Refresh only reads status. Reopening Export Form starts a new export request.

If preparation succeeded but saving failed or was canceled, retry the download rather than rebuilding the package. On web, check browser downloads; desktop uses a save dialog, and mobile uses a share sheet.

Import a Form

1. Open the intended Library folder.
2. Select More Library actions > Import Form.
3. Review Destination.
4. Select Choose .form archive and choose a file no larger than 25 MiB (26,214,400 bytes).
5. Review the proposed name, section/field/option/image counts, assets, and warnings or blockers.
6. When Ready to import appears, select Import as new Form.

CareMap opens the new Form in the editor. It has new identities and uses the destination's access rules. Existing content is not replaced. A duplicate name receives a suffix such as (2).

Changing Destination keeps the uploaded archive and automatically requests a new preview. Wait for that result before confirming. If preview fails, use Retry to inspect the same upload again.

Media requirements

Every authored media asset must be bundled and independently readable. Missing, inaccessible, unsupported, or incomplete media blocks import. Export a complete source Form again rather than proceeding with a partial archive.

The preview describes the proposed copy; it does not create responses or Reports. Review the imported Form's layout, answer keys, and media before use.

If a step fails

Confirm destination access, workspace authoring status, capacity, and archive size. A package may expire while waiting; select the local archive again when the error requests it.

If export history cannot refresh, use Refresh after reconnecting. A history error need not invalidate an already prepared archive.

After a lost import response, retry confirmation in the same dialog with the same uploaded archive and destination. If the import already committed, CareMap recovers that Form instead of creating another. Current destination and Form access are still required. Selecting and uploading the archive again starts a separate import; check Library first if the original dialog is gone.
