Import and export Forms
Move supported Form definitions into or out of CareMap.
Use a .form archive to move an authored Form between CareMap workspaces or
keep a portable copy of its definition. Importing creates a new, independent
Form; it does not replace the source Form.
What a Form archive contains
A Form export includes the Form's sections, fields, choices, quiz settings, answer keys, cover, display images, and Record-binding details. It does not include submissions, answers, fill history, or Reports.
Because answer keys are included, you need edit access to export a Form. Importing requires edit access to the destination folder. Only folders where you can add content are offered as destinations.
Export a Form
- In Library, find the Form you want to move.
- Open the Form's More menu, then select Export Form.
- Wait while the export changes from Preparing to succeeded.
- Review the archive size and packaged-asset count.
- Select Download .form and save the archive to your device.
The export dialog also shows recent package history. If preparation fails and a retry is available, select Retry. The browser, desktop app, or mobile device may ask where to save the downloaded file.
Import a Form
- Open the destination folder in Library.
- Open More Library actions, then select Import Form.
- Choose the Destination folder.
- Select Choose .form archive, then choose a
.formfile no larger than 25 MB. - Review the proposed Form name and the counts for sections, fields, options, images, Record bindings, and bundled assets.
- Read every warning or blocker. Continue only when the preview says Ready to import.
- Select Import as new Form.
CareMap opens the imported Form in the editor. The source remains unchanged, and the imported copy receives its own identity. If the destination already has content with the same name, the preview shows the adjusted name before you import.
Media and Record bindings
Cover and display images must be present in the archive. CareMap blocks the import when required media is missing, unreadable, or unsupported so that it does not create a partly broken Form.
Record bindings are matched against compatible Record Types in the destination workspace. When a binding has no single compatible match, the preview warns that the binding will be skipped. The rest of the Form can still be imported; review and reconnect that field in the editor afterward.
Fix common problems
- Export Form is missing: You need edit access to the Form. Read-only access is not enough to export answer keys.
- Import Form is missing or no destination is available: Open a folder where you have edit access.
- The archive is rejected: Choose an unmodified CareMap
.formarchive and confirm that it is no larger than 25 MB. - The preview is Blocked: Replace the archive with a complete export whose cover and display images are bundled, then preview it again.
- A Record binding warning appears: Import the Form, then connect the field to a compatible Record Type in the destination workspace.
- Export preparation failed: Use Retry when offered. If it fails again, close the dialog, confirm the Form's media is available, and try a new export.
