Build a Form
Create Form sections and arrange fields or supporting content.
Use the Form builder to organize questions and supporting content into ordered sections. You need owner or edit access to the Library folder and Form authoring through an active AT or Pro license.
Create a Form
- Open Library and enter the folder where the Form should live.
- Open the Create menu, then select New Form.
- Enter a Form name and select Create Form.
CareMap opens the new Form in the editor with Section 1 ready to configure. Use Form settings to change the name or description, choose a cover image, set the default section background, or configure quiz scoring.
Add sections, fields, and content
- Select the current section and edit its title and optional description. Use Section settings when the section needs its own appearance.
- Select Add field or content.
- Under Fields, choose the answer type you need: Checkbox, Short text, Long text, Number, Date & time, Single choice, Multiple choice, or Upload. Under Content, choose Info text or Display image for instructions that do not collect an answer.
- Configure the item's label, help text, answer rules, choices, upload limits, Required state, or other available settings, then select Done.
- Open the Section menu and select Add section when the Form needs another step.
Use the outline to switch sections. Section and item menus let you move, duplicate, edit, or delete content. You can also use the reorder handles; items can be moved to another section. At least one section must remain in the Form.
Save and preview safely
There is no separate Save button. CareMap autosaves edits after a short pause and saves immediately for actions such as adding, deleting, or reordering. Wait for Saving… to finish before leaving the builder. Undo and Redo are available for recent builder changes.
Select Preview to test the current Form without creating a response or Report. If a numbered Form issues button appears, open it and select each message to jump to the setting that needs attention. A Form must have a name, usable sections and items, complete field choices or upload rules, and properly configured images and quiz answer keys before it can be filled.
Forms do not have draft, publish, or version states. Every successful autosave updates the current Form, so preview your changes before other people use it.
Fix common problems
- Save failed: Read the displayed message. Check your connection, and if another editor changed the Form, reload before continuing and reapply your change.
- Preview or Fill is unavailable: Open Form issues and resolve every item, then wait for autosave and try again.
- Editing is paused: If the workspace has reached its storage allowance, return to Library and remove unneeded content or files before continuing.
- Actions are missing: Confirm that you have edit access to the folder and that Form authoring is available for your license.
