Care records and observations
Find a Person's goals, tasks, documents, and observation timeline, and understand their separate access rules.
Care records brings together goals, tasks, supporting documents, and recorded observations for one Person.
Open the care record
- Open People > Clients, search for the Person, and select their row.
- Check the name and relationship context to confirm the subject.
- Select Care records.
- Open or update the section you need, following its own access messages.
| Section | Use it for |
|---|---|
| Goals | Outcomes, review dates, and task links |
| Tasks | Care work, priorities, due dates, and completion states |
| Documents | Care artifacts, references, downloads, and acknowledgments |
| Observations | A timeline of recorded Report answers and available CareMap profile observations |
The separate Documents tab holds Profile documents, including ISP, SF Assessment, SF Plan of Care, and Service Plan. Uploading one of those documents does not automatically create Goals or Tasks.
Read the observation timeline
Scroll to Observations. Entries show a source badge, summary, source label, and observation date, with the newest observations first. The item count is the number of entries available in this timeline.
Use Report on a Report entry to open its source. Use Profile on a CareMap entry to return to the Person's Profile. These controls may lead to additional permission checks. The timeline is for reading recorded observations; it has no Add observation or inline answer editor.
Consent restrictions can replace an observation's summary with an explanation and hide its value. Read the warning and ask the responsible administrator to review the relevant consent record. Uploading a file named Consent does not activate consent. See Consent and access.
Why sections can differ
Goals and Tasks support the Person's linked account, active care-team access, qualifying organization access, and administrators. Documents also support effective Guardian access. The observation timeline requires care-team, qualifying organization, or administrator access; being the linked account or a Guardian alone does not unlock that timeline.
CareMap profile observations additionally require access to that profile's questionnaire. Report entries depend on source visibility. An empty timeline therefore does not prove that no work has ever been recorded.
If a section fails to load, use its Try again control. A missing or denied section needs its own access reviewed even when another section opens. Refresh or reopen the profile after access changes. Archived People can retain care history, while new actions remain restricted by the Person and item statuses.
