Manage care plan goals
Record outcomes and review dates, update goal status, and connect goals to care tasks.
A care plan goal records an intended outcome for one Person and can collect related care tasks.
Open People > Clients > the Person > Care records. The linked account, authorized active care-team members, qualifying organization Staff, and CareMap administrators can manage these goals. Guardian status by itself does not grant goal access; see Care records.
Add a goal
- In Goals, enter a Title.
- Optionally describe the Outcome you want to achieve.
- Optionally choose a Review date.
- Choose Status: Active, Paused, Achieved, or Archived.
- Select Add goal and confirm the new row appears.
The initial status is Active. A review date records when to revisit the goal; adding it does not create a scheduled reminder. Use Schedule for scheduled work.
These goals are separate from an uploaded SF Plan of Care or Service Plan. Uploading a Profile document does not populate the Goals list.
Update, archive, or restore
Use the goal row's status selector or review-date field. Each change saves immediately; there is no separate Save button. Clear the date to remove it. The row displays the title and outcome but does not provide inline editing of those fields.
Choose Achieved when the outcome is reached, or Paused when work is on hold. Select the archive icon or Archived to retain the goal in the list as archived. To restore it for an active Person, select the appropriate status again. Archiving a goal does not archive or complete its linked Tasks.
For an archived Person, goals can be recorded or moved to Achieved or Archived. Creating or updating a goal as Active or Paused is blocked. A CareMap administrator can restore the Person when active care should resume.
Understand task counts
Add a task and choose this goal under Goal; see Manage care tasks. Task changes refresh the goal counts.
The section's “active” count means all goals except Archived, so Paused and Achieved goals are included. A goal's “active / tasks” count similarly means non-archived tasks out of all linked tasks. Done tasks remain in that active count until archived; it is not a count of unfinished tasks or a completion percentage.
If a save fails, read the error and use Try again to reload. Check the Person's status and your current relationship before retrying. After an interrupted request, confirm whether the goal was saved before adding it again.
