Guide-to-Form mapping compatibility
Choose compatible Form field types and understand shared targets, audio limits, and final validation.
A Field block maps to one Form field, and every answer method in that block must be compatible with the target. Use this reference when Target field does not offer the field you expected.
For the save workflow, see Link Guide answers to a Form.
Supported combinations
| Guide method | Compatible Form target |
|---|---|
| Type | Short text or Long text |
| Date & Time | Short text or Long text |
| Number, or an older Scale method | Number |
| Options | Short text, Long text, Single choice, or Multiple choice |
| Photo | Upload configured to accept images |
| Audio Recording | Upload configured to accept audio; text targets have the limitation below |
| Completion | Checkbox |
| Order, Sort into categories, Signature, Whiteboard, Skip | No supported Form mapping |
For example, Type and Options can share a text target. Type and Number cannot share a target because they need different Form field types. Use separate Field blocks when their responses need different destinations.
Date & Time maps its saved readable answer into text. It does not map directly to a Form Date, Time, or Date and time field.
Completion maps the confirmation value. Supporting notes and photos are not additional answers to the Form's Checkbox field.
Options and Other
For a choice target, explicitly map every final Guide option to a target option. Multiple-selection Guide answers cannot map to Single choice. If the Guide method allows Other, the target choice field must also allow Other.
For a text target, the saved answer summary becomes the text; option-to-option conversion is unnecessary.
A deleted target option or a new unmapped source choice can make the mapping invalid. Repair and save it before collecting new responses.
Recorded audio is not text
The selector also permits Audio Recording to map to a text field for retained transcript-style answers. A newly recorded or uploaded audio response is an audio file, not a transcript. That response cannot complete a text mapping.
Use an audio-enabled Upload target for recordings, or collect a separate Type answer in its own mapped Field. Selecting a text target does not enable automatic transcription.
Shared targets and final checks
Several Field blocks or saved responses can feed one target field. CareMap selects the newest compatible answer for that target, then validates the result against the Form.
This does not mean it chooses an older valid answer whenever the newest fails a length, range, selection, or upload rule. Conversion errors and final Form validation still stop completion. Ensure every required Form field receives a suitable answer, even if the corresponding Guide Field was optional.
Mapped media must be ready and remain available. The Form's allowed media kinds, file formats, extensions, and other upload rules still apply.
Include in reports controls the Guide answer entry. It does not remove the response from mapping candidates. The combined Report may therefore contain the mapped Form answer even when the Guide method's report inclusion is off. Review both settings when deciding what to collect.
