Upload fields
Use an Upload field to collect one or more permitted files.
An Upload field lets a person attach files while filling a Form. Use it for documents, photos, recordings, or other evidence that should stay with the submitted Form report.
Add an Upload field
You need edit access to the Form.
- Open the Form from Library, then select the section that needs the field.
- Select Add field or content.
- Under Fields, select Upload.
- Enter a clear Label and optional Help text.
- Keep Required on when at least one file must be attached.
- Under Allowed files, select at least one of Image, Audio, Video, or File.
- Add optional Accepted MIME types or Accepted extensions when the field needs a narrower list.
- Turn on Allow multiple files when needed, then set Maximum files.
- Select Done, then use Preview to check the field.
A Form cannot be filled when an Upload field has no allowed file category. Upload fields remain in Reports and cannot write files into a Person Record.
Limit the files people can choose
The allowed categories are the broadest rule. Use File for documents and other non-media files. Optional filters narrow the category further:
- Enter MIME types as a comma-separated list, such as
image/png, application/pdf. - Enter extensions without a leading dot, such as
png, pdf, docx. CareMap also accepts them when a dot is included.
Leave either filter blank when it should not add another restriction. When both filters contain values, an upload must satisfy both.
Attach files while filling
- Select Choose files and choose files permitted by the field.
- Wait while the field shows Uploading….
- Confirm that the field shows the expected number of files ready.
- Continue to the next section or select Submit Form.
CareMap disables navigation while an upload is in progress. Selecting files again replaces the current selection for that field. A required Upload field must have at least one ready file before you can continue.
Fix common problems
- A file cannot be selected or is rejected: Check its category, extension, and file type against every limit shown for the field.
- Too many files were selected: Choose files again and stay within the Maximum files value.
- The upload fails: Retry the selection. If CareMap reports a storage limit, use a smaller file or free workspace storage before trying again.
- The wrong file is ready: Select Choose files again and choose the correct replacement before submitting.
