Link Guide answers to a Form

A mapped Guide sends compatible Field answers into one target Form. Successful completion creates one Form submission and a combined Report.

You need Guide edit access and a workspace that allows authoring. The target must be an accessible active Form; Form edit access is not required.

Select the target

1. Open Guide > Guide settings > Form mapping.
2. Choose Target Form, or No Form target to clear the selection.
3. Select Save Form target.
4. If replacing or clearing an existing target, review Change target Form? and confirm Change Form only when ready to clear its Field mappings.

This saves separately from the other Guide settings. Cancel does not undo a saved target change. Reset Form selection abandons only the unsaved selection. Save or reset it before using Save guide.

Map a Field

1. Edit a Field block and configure its answer methods.
2. Open Linking and choose Target field.
3. For a Form choice field, complete Option conversion for every final source option.
4. If block edits are pending, select Save block and continue.
5. Select Save Form mapping.
6. Reopen Linking to review the saved mapping and test the Guide.

Mapping uses saved methods. Saving the block alone does not save the mapping. Saving the mapping applies it independently; discarding later block edits does not undo that saved link.

Only fields compatible with every method in the block are offered. A mixed Field may therefore have fewer choices, or none. See the compatibility rules.

For nested Options, map the final choices, not their parent categories. Multiple sources can map to a shared target choice when appropriate. If an option was removed from the Form, select an available replacement explicitly; CareMap does not choose one automatically.

Change or clear a mapping

Choose another Target field and save, or select Not mapped and Save Form mapping to clear the link.

An existing mapping can prevent saving incompatible method changes. Clear the mapping first, save the revised block, then choose and save a compatible mapping. Clearing is available even when the block has pending edits.

Changing the Guide's target Form clears all its Field mappings. Rebuild them for the new target; canceling the settings dialog does not restore them.

Resolve a blocked save or completion

• No Form target: Select and save one in Guide settings first.
• Unavailable target or option: Restore compatible methods, choose an available replacement, or clear the mapping.
• Block or Form changed: Reopen the selector and review the current definition before saving again.
• Archived target: Restore the Form through its normal Library controls, or choose another active Form and remap.
• Completion fails: Check the target's required fields, validation, option conversions, and media readiness. A saved link is not a guarantee that every possible answer satisfies the Form.

When several mapped responses target the same Form field, the newest compatible answer wins. Earlier Guide responses can still appear in their own Report entries.
