Information text in Forms

Write, format, save, and arrange Form instructions, including safe use of Markdown Source mode.

Use Info text for formatted instructions and explanations between Form questions. It displays content without collecting an answer or contributing to a score.

Add or edit instructions

You need Form edit access in a workspace that allows authoring.

  1. Open the Form builder and select the section.
  2. Select Add field or content, then Info text under Content.
  3. In Edit Information text, replace the starter content in Information text (Markdown).
  4. Use Rich text and the formatting toolbar to arrange the instructions. Add headings, emphasis, lists, links, or a table as needed.
  5. Finish any image insertion, then select Done. Wait for the item to save.
  6. Use Preview to check the text alongside its questions.

For an existing item, open Edit from its actions. New Info text is added to the Form when you finish the item, so abandoning a new item does not leave an empty saved question behind.

Format readable instructions

The toolbar supports bold, italic, underline, inline code, bulleted and numbered lists, checklists, links, tables, headings, quotes, code blocks, and separators. Use the alignment control to position the text. Inside a table, Add row, Add column, and Remove table become available.

These are display instructions: a checklist inside Info text is not a Form response. Use a Checkbox or choice field when an answer must be recorded.

Use Insert image for a picture within the instructions; follow inline images for sources, captions, sizing, and failed insertions. A separate Display image is useful when the image should be its own movable Form item.

Use Source mode carefully

Source mode lets you edit Markdown. If the current formatting cannot be fully represented in Markdown, CareMap displays a warning and makes the source read-only until you select Enable source editing.

Viewing the source alone does not replace the formatted content. Editing it can simplify formatting. Return to Rich text and inspect the result before selecting Done. Keep complex formatting in Rich text when you do not want that conversion.

Save, discard, or arrange content

Closing a changed item, including selecting Cancel, offers Save changes, Discard changes, or Keep editing. Choose deliberately; closing is not an automatic discard. Done also saves. If saving fails, keep the editor open and follow Form saving.

Use the reorder handle to move the item within its section. Its actions also provide Duplicate, Move to section, and Delete; see sections and ordering.

Blank Info text does not block readiness, but it gives the reader no useful instructions. Remove unused items. If a long item cannot save, shorten it: information text is limited to 100,000 characters of Markdown, including formatting.
