Checkbox fields

A Checkbox field collects a yes-or-no answer. Use it for a confirmation, agreement, completed step, or any statement that is either checked or unchecked.

This field shows one checkbox rather than a list of choices. Use a Multiple choice field instead when people should be able to select several named options.

Add and configure the field

You need edit access to the Form.

1. Open the Form from Library, then select the section that needs the field.
2. Select Add field or content.
3. Under Fields, select Checkbox.
4. Enter a clear Label and optional Help text.
5. Edit Checkbox label to the statement shown beside the box. New Checkbox fields use Yes by default.
6. Keep Required on when the person must check the box before continuing or submitting.
7. Select Done, then use Preview to test the field.

Write the checkbox label as a complete, specific statement, such as “I confirm that the information above is correct.” Avoid using an unclear label such as “Yes” when the question or agreement would no longer be visible beside it.

Required and optional checkboxes

A required Checkbox field is a confirmation: the person must check it. Leaving it unchecked blocks the next step or submission and CareMap identifies the field that still needs attention.

An optional Checkbox field can remain unanswered. CareMap summarizes a recorded checked answer as Yes and a recorded unchecked answer as No in the Form response and report.

Grading and Records

When a quiz mode is enabled in Form settings, the field can be Ungraded or Automatically graded. Automatic grading compares the answer with a configured Correct answer of Yes or No.

A required Checkbox field can only use Yes as its correct answer because a required field cannot be submitted unchecked. Make the field optional when No needs to be a valid or correct response.

When Person Record targeting is available, the author can connect the field to a compatible Boolean Record Type so the submitted value is copied to the selected person's Record.

Fix common problems

• The person cannot continue or submit: If the field is Required, make sure the checkbox is checked. Otherwise, turn off Required when No should be allowed.
• The checkbox wording is unclear: Edit Checkbox label so the statement beside the box makes sense by itself. Use Help text for extra context.
• No correct answer is available: Enable a quiz mode in Form settings, choose Automatically graded, and select Yes or No.
• No compatible Record target appears: Only Boolean Record Types are offered for Checkbox fields.
