Short text fields
Use a Short text field to collect a brief one-line answer.
A Short text field collects a brief written answer in a one-line input. Use it for names, short labels, identifiers, quick notes, or any answer that should stay compact.
Use a Long text field instead when people need room for paragraphs or a longer explanation.
Add and configure the field
You need edit access to the Form.
- Open the Form from Library, then select the section where the question should appear.
- Select Add field or content.
- Under Fields, select Short text.
- Replace the default Label with the question or prompt people should answer.
- Optionally add Help text for context, examples, or formatting instructions.
- Optionally add a Placeholder to show example text before the person answers.
- Optionally set Minimum length or Maximum length.
- Choose whether the field is Required, then select Done.
- Use Preview to test the field before sharing or filling the Form.
New Short text fields start as required. The placeholder is only a hint; it is not submitted as the person's answer.
Set length rules
Use Minimum length when an answer must include at least a certain number of characters. Use Maximum length when the answer should stay within a specific size.
The minimum cannot be greater than the maximum. Leave either field blank when that side of the length rule should not be limited. CareMap checks length rules before saving the Form configuration and again when a response is submitted.
Fill, grade, and report the field
People filling the Form type into a single-line text input. A required field must contain non-blank text before the Form can be submitted. If length rules are configured, the answer must fit those rules too.
When a quiz mode is enabled in Form settings, the field can be Ungraded or Automatically graded. Automatic grading uses one Correct answer. CareMap normalizes the submitted and correct answers by trimming them and collapsing repeated whitespace. By default, uppercase and lowercase are treated as the same; turn on Match uppercase and lowercase exactly when capitalization must match.
The submitted text is included in the Form report. When Person Record targeting is available, the author can also connect the field to a compatible text Record Type so submitted values are copied to the selected person's Record.
Fix common problems
- The field will not save: Check that Minimum length is not greater than Maximum length.
- A person cannot submit: Check whether the field is Required and whether the answer satisfies any length rules.
- The placeholder did not appear in the report: Placeholder text is only a hint. The person must type an answer for text to be submitted.
- A quiz answer is marked wrong: Compare it with the Correct answer. Automatic grading trims extra space and ignores capitalization unless Match uppercase and lowercase exactly is on.
- A Record Type is not available: Only compatible text Record Types are offered for Person Record targeting.
