Multiple choice fields

A Multiple choice field collects one or more selections from a list. Use it when several answers can be true at the same time, such as symptoms, preferences, supplies needed, or tasks completed.

Use a Single choice field instead when the person must choose exactly one answer, or a text field when the answer should not come from a fixed list.

Add and configure the field

You need edit access to the Form.

1. Open the Form from Library, then select the section where the question should appear.
2. Select Add field or content.
3. Under Fields, select Multiple choice.
4. Replace the default Label with the question or prompt people should answer.
5. Optionally add Help text for context, examples, or instructions.
6. Under Choices, edit the option labels. Use Add choice for more options, or use the option actions to delete or reorder them.
7. Keep Required on when the person must answer before submitting the Form.
8. Optionally turn on Allow Other to include a write-in response.
9. Optionally set Minimum selections or Maximum selections.
10. Select Preview to test the field before sharing or filling the Form.

Multiple choice fields display as checkboxes while the Form is being filled.

Selection limits

Selection limits count each checked choice as one selection. If Allow Other is on, a filled Other response also counts as one selection.

• Required means the person must select at least one choice or enter a non-empty Other response.
• Minimum selections requires at least that many selections when the field is answered.
• Maximum selections prevents submitting more than that many selections.
• If the field is optional, a person can leave it blank. Keep Required on when a minimum selection count should make the field mandatory.

The minimum cannot be greater than the maximum. CareMap also shows a Form issue if the field has no choices and Allow Other is off, or if any choice is left without a label.

What people see while filling the Form

People see the field label, any help text, and a checkbox for each choice. If the field is required, CareMap marks it with the required indicator. When Allow Other is on, selecting Other opens a text box for the write-in answer.

CareMap checks the required state and selection limits before the Form can be submitted. Blank Other text is ignored, so the person must type a response for Other to count.

Grading and reports

When quiz mode is off, Multiple choice answers are collected without grading. When the Form uses a quiz mode, you can set the field's Grading to Automatically graded and choose one or more Correct answers from the configured choices.

For automatic grading, the selected choices must match the answer key exactly. Selection order does not matter, but missing choices, extra choices, or an Other write-in response make the answer incorrect.

Submissions, reports, and exports summarize Multiple choice answers with the selected choice labels. If the person entered an Other response, that text is included in the answer summary too.

Fix common problems

• The Form says to add a choice: Add at least one choice, or turn on Allow Other if the field should only collect write-in answers.
• A choice issue appears: Fill in every choice label. Empty choice labels block the Form from being filled.
• The person cannot submit: Check whether Required, Minimum selections, or Maximum selections is blocking the answer.
• Other is not saved: Make sure Other is selected and the text box has a non-empty response.
• A quiz answer is marked wrong: Compare the selected choices with the answer key. Automatic grading requires the same set of configured choices and does not treat Other text as a correct answer.
