Multiple choice fields

A Multiple choice field lets someone select several named answers, such as symptoms, preferences, or supplies. It displays checkboxes. Use Single choice when only one answer is allowed.

Add and configure the field

You need edit access to an active Form in an authoring-enabled workspace.

1. In the builder, select the section that needs the question.
2. Select Add field or content > Fields > Multiple choice.
3. Enter a Label and optional Help text.
4. Edit the two starter choices. Use Add choice, Delete choice, and the reorder handles to arrange them.
5. Review Required and turn on Allow Other if write-in answers are allowed.
6. Optionally set Minimum selections and Maximum selections.
7. Select Done, wait for saving, and test Preview.

New fields start required. Choice handles support dragging or keyboard pickup/drop with Space or Enter, movement with arrows/Home/End, and Escape to cancel. Field-dialog changes wait for Done; see item saving for cancellation.

Set achievable selection limits

Each checked choice counts as one. A selected Other response with nonblank text counts as one more.

• Required needs at least one completed selection.
• Minimum selections applies when an optional field is answered; leaving it completely blank is allowed.
• Maximum selections limits the accepted answer. Extra choices can still be checked, but the person must remove them before continuing.
• Minimum and maximum cannot contradict each other. Ensure the available choices can actually satisfy the minimum.

The minimum accepts whole numbers from 0–500; the maximum accepts 1–500. A minimum of zero does not cancel Required. To require exactly two answers, turn on Required and set both limits to two.

Every choice needs a label. Include at least one named choice or allow Other.

Answer the question

Check the applicable choices. Select Other to open its text box. Once selected, Other requires text even for an optional question or when other choices are already checked. Enter text or uncheck Other to continue. Unchecking Other clears its write-in text.

CareMap shows the selection limits and highlights invalid answers. An optional question may be skipped entirely, but a partially answered question must satisfy its limits.

Configure grading

Enable Quiz mode in Form settings, choose Automatically graded, and select one or more Correct answers. The key must satisfy the selection limits.

The submitted set must match exactly. Order does not matter; a missing choice, extra choice, or Other response makes the field incorrect. There is no partial credit per option. Selected labels and Other text appear in the response.

Clear a choice's correct-answer selection before deleting it. An old key can prevent saving even after switching to Ungraded. Follow answer-key maintenance if saving fails.
