Documents and signatures

Work with supported documents and visible signature information.

Documents keep files and document references with a person's Care records. They can also show typed acknowledgments that identify who reviewed an available document.

Add a document

  1. Open People, select the person, and open Care records.
  2. In Documents, enter a Title and choose a Type: Care artifact, Consent, Report export, or External.
  3. Optionally enter a Source and Notes.
  4. Under File, choose a PDF, text, PNG, JPEG, GIF, or WebP file up to 25 MB.
  5. Select Upload document.

If you leave File empty, a title is required and the action becomes Add document. This creates a document reference without an uploaded attachment.

CareMap can also add Documents from supported consent and Report export activity. Their source labels and available actions may differ from a file you upload yourself.

Upload several files

Select Bulk in the Documents heading, then:

  1. Choose the shared Type, Source, and Notes.
  2. Select multiple supported files and review the title generated for each.
  3. Optionally select Acknowledge each upload and enter the signer details.
  4. Select Start upload.

Each file finishes independently. A failed item does not stop later files. Correct its title, file, or connection problem, then select Retry failed. Reset clears the bulk selection after the upload is finished.

Review and acknowledge a document

Each row shows its title, type, date, source, notes, and availability. Depending on the source and state, you can select Download, open its Report, or select Extract to request review text. Extraction can be pending, skipped, failed, or successful; read the displayed status because extraction is not available for every file or CareMap environment.

For an available document, enter a Signer name and optionally a Relationship and Statement, then select Acknowledge. CareMap records the signer and time and can show Signed by you, active signature counts, and revoked or superseded history. A new acknowledgment by the same account supersedes that account's previous active one.

An acknowledgment records that someone reviewed the document. It does not draw a signature on the file, change the attachment, or by itself grant access.

Understand status and access

  • Available documents can expose their allowed download and acknowledgment actions.
  • Pending means the file or source is still being prepared.
  • Unavailable or Redacted means the content or an action is restricted; the row explains why when CareMap can show a reason.
  • Archived documents remain part of the history but cannot be acknowledged or extracted. An archived person cannot receive new Documents.

You see Documents only for a person you are allowed to access. Consent, Report, care-team, Guardian, or organization access can also affect availability. If the section fails to load, select Try again. If an expected action remains missing, confirm the document is Available and ask the appropriate person or workspace manager to review your access.
